Summer Work
Summer Work is distributed at the end of each school year. It is available for pick-up for new families and/or those who need it through Kate Terry at kterry@broadwateracademy.org.
Lower School Summer Work (K - 4) - can be picked up through Kate Terry at kterry@broadwateracademy.org.
Middle School Summer Work Information (Grades 5 - 8)

Medical Forms
Virginia School Entrance Form- PK, Kindergarten, & any new students from another State
PK Medication Consent Form - for Pre-K students who need medication during school hours
Medication Consent Form - (Kindergarten - Grade 12) any student who will need prescription medication during school hours
Athletic Physical Form - any Grade 5 - Grade 12 student who would like to participate in athletics
Allergy Plan - any student who has an allergy which can lead to anaphylaxis
Asthma Action Plan - any student who needs an inhaler or respiratory medication
PLEASE PROVIDE THE NURSE WITH AN UPDATED IMMUNIZATION RECORD ( Please note there have been changes to school immunization requirements as of July 1, 2021.)
